Patient Triage & Prioritization
We assess your active patient panel by acuity — identifying who needs immediate attention, who needs a warm handoff, and who can self-transition to a new provider.
Referral & Care Transition
Clinical summaries, medication reconciliation, and warm referrals to receiving providers. We ensure continuity for your most complex patients.
Telehealth Bridge Visits
Interim virtual visits for patients who need clinical attention during the transition period. Same-day available for urgent needs.
Compliance Documentation
We create the clinical closure file — proof of lab follow-up, patient notification documentation, and transition records that protect you from future claims.
