How we maintain high standards of care, protect your data, and ensure continuity of service.

1. Quality Improvement Program
InnoCre is committed to continuous quality improvement. We regularly evaluate our clinical processes, patient outcomes, and patient satisfaction to identify opportunities for improvement. Our quality improvement activities include:
- Regular review of clinical outcomes and treatment effectiveness
- Patient satisfaction surveys and feedback collection
- Analysis of complaint trends and incident reports
- Provider credentialing and ongoing competency evaluation
- Adherence to evidence-based clinical guidelines and best practices
- Periodic review and update of policies and procedures
2. Clinical Records Management
- Your medical records are maintained in a secure, HIPAA-compliant electronic health record (EHR) system
- Records are retained for a minimum of 10 years from the date of last treatment, or longer as required by applicable state law
- You have the right to request access to your medical records at any time. Requests can be made by contacting atulsv@innocre.org
- Record requests will be fulfilled within 30 days, or sooner as required by state law
- A reasonable fee may apply for copies of records, as permitted by law
3. Provider Credentialing
All Innocre providers are:
- Licensed and in good standing in each state where they provide care
- Board-certified or board-eligible in their specialty
- Subject to initial credentialing and re-credentialing every three years
- Required to maintain current malpractice insurance
- Verified through the National Practitioner Data Bank (NPDB) and primary source verification
4. Data Security & Breach Notification
- We implement administrative, physical, and technical safeguards to protect your health information in compliance with HIPAA and applicable state laws
- All telehealth visits are conducted over encrypted, HIPAA-compliant platforms
- Access to patient data is restricted to authorized personnel on a need-to-know basis
- In the event of a data breach involving your protected health information, we will:
- Notify affected individuals within 60 days of discovery, or sooner as required by state law
- Provide a description of the breach, the types of information involved, and the steps we are taking
- Notify the U.S. Department of Health and Human Services as required by HIPAA
- Offer guidance on steps you can take to protect yourself
- If you suspect unauthorized access to your health information, contact us immediately at atulsv@innocre.org
5. Continuity of Care
- If your provider becomes unavailable (due to illness, leave, or other circumstances), Innocre will make reasonable efforts to arrange care with another qualified provider or refer you to an appropriate external provider
- If Innocre ceases operations, patients will be notified at least 30 days in advance and provided with instructions for obtaining their medical records and transferring care

- For medical emergencies, always call 911 or go to your nearest emergency room
6. After-Hours Care
Innocre currently provides care during scheduled appointment hours. If you have an urgent medical need outside of our operating hours:
- For emergencies, call 911
- For non-emergency urgent needs, you may leave a message at (844) 836-2669 and we will respond within 24 hours
- You may also visit your local urgent care center or emergency room
